Care To Heal is an aged care and disability services provider committed to delivering respectful, person-centred support that helps people remain safe, independent and connected within their homes and communities.
We are expanding our allied health services and are seeking a qualified and compassionate Physiotherapist to provide in-home services to aged care clients.
Current service areas include Dandenong, Noble Park and surrounding SE suburbs, with opportunities to support clients across additional Melbourne locations as new referrals are received.
About the Role
The Physiotherapist will provide assessment, treatment, rehabilitation and ongoing clinical support to older people in their homes.
The role is flexible and referral-based. Referrals will be offered according to client needs, location, practitioner availability and the agreed service arrangement.
Key Responsibilities
- Conduct physiotherapy assessments in clients’ homes
- Assess mobility, strength, balance, pain and falls risk
- Develop individualised treatment, rehabilitation and exercise programs
- Provide falls prevention and risk-reduction strategies
- Support clients to maintain or improve mobility and independence
- Provide pain management and reablement-focused interventions
- Recommend mobility aids, assistive equipment and environmental supports
- Educate clients, families and support workers about safe mobility and exercise
- Prepare accurate clinical notes, reports and progress updates
- Communicate with Care To Heal, care managers and other health professionals
- Participate in care planning and multidisciplinary communication where required
- Maintain privacy, confidentiality and professional boundaries
- Deliver services in line with professional standards, aged care requirements and client goals
Essential Requirements
Applicants must have:
- A recognised qualification in Physiotherapy
- Current AHPRA registration as a Physiotherapist
- A current police check or an acceptable worker screening clearance
- A valid Australian driver’s licence
- Access to a reliable and appropriately insured vehicle
- Strong clinical documentation and communication skills
- The ability to work independently in clients’ homes
- A commitment to person-centred, respectful and culturally safe care
- An understanding of privacy, consent, dignity of risk and professional boundaries
Desirable Experience
- Experience in aged care, community health or home-based physiotherapy
- Experience supporting older people with reduced mobility or chronic health conditions
- Experience in falls prevention, pain management and rehabilitation
- Knowledge of wellness and reablement approaches
- Experience recommending mobility aids and assistive equipment
- Familiarity with aged care service delivery and multidisciplinary care
- Experience preparing reports for care providers or care managers

Engagement Arrangement
This position is intended as a contract or subcontract opportunity. The final arrangement, service rate, travel payment, invoicing process and referral expectations will be confirmed in a written agreement.
Applicants engaged as independent contractors will generally be required to:
- Maintain an active ABN
- Provide valid insurance documentation
- Issue compliant tax invoices
- Maintain professional registration and screening requirements
- Manage their own taxation and contractor obligations
