Your Role
We are seeking a compassionate and dedicated Case Worker to join our Wickham team and provide trauma informed, person-centred support to individuals and families experiencing homelessness or at risk of homelessness. This is an opportunity to make a meaningful impact by supporting some of the most vulnerable members of the community to achieve safe, stable and sustainable housing outcomes. Working within a collaborative and supportive environment, you will deliver high-quality case management, advocate for clients, build strong community partnerships, and help people overcome barriers to independence and wellbeing.
Your Responsibility
Deliver trauma informed case management and support to people experiencing homelessness or at risk of homelessness.
Develop, implement, monitor and review client case plans in line with SHS guidelines and outcomes frameworks.
Support clients to access and maintain safe, stable and sustainable accommodation.
Attend to clients' psychosocial needs and facilitate referrals to appropriate support services.
Conduct home visits with clients living in temporary, transitional and permanent accommodation.
Undertake assertive outreach activities and engage with vulnerable members of the community.
Build and maintain collaborative partnerships with government agencies, community organisations and key stakeholders.
Conduct risk assessments and support clients to manage risks and achieve their goals.
Maintain accurate and timely client records, case notes, statistics and reporting requirements.
Supervise volunteers and students on placement where required.
Participate in team meetings, training and continuous improvement initiatives.
Promote a safe, inclusive and welcoming environment for clients, staff and volunteers.
Ensure compliance with organisational policies, service delivery frameworks, workplace health and safety requirements, and risk management processes.
You Will Need
Minimum Certificate IV in Community Services, Welfare, or a related discipline, or currently undertaking tertiary studies in a relevant field.
Previous experience supporting marginalised and vulnerable individuals, particularly people experiencing homelessness or housing insecurity.
Demonstrated understanding of the complex issues contributing to homelessness, including family breakdown, domestic and family violence, poverty, disability, mental illness, and alcohol, drug or gambling addictions.
Knowledge of local community support services, government agencies and non-government organisations that support people experiencing homelessness.
Proven ability to build and maintain effective relationships with service providers and community stakeholders.
Excellent written and verbal communication skills.
Strong computer literacy and experience maintaining accurate client records and case notes.
Ability to work both independently and collaboratively within a team environment.
Sound judgement and the ability to respond appropriately in crisis and emergency situations.
Understanding of Work Health and Safety legislation and commitment to safe work practices.
Current NSW Driver Licence.
