About the role..
Senior Helpers Northern beaches and Suburbs are seeking an experienced Care Partner to join our Care team. This is an office based role in our Northern beaches office.
Primary Responsibilities:
- Conduct holistic client assessments and develop care plans that support their needs, maximise client choice, well-being and independence.
- Conduct regular monitoring of services and care plan reviews to ensure optimal care and services are maintained to meet changing needs
- Co ordinate services and organise supports based on government compliant care plan for the client
- Communicate effectively and build trust with clients and their family or representatives.
- Responsible for management of your own portfolio of clients for their care, SAH budgets and all their relevant documentation
- Liaise and work closely with internal team members like RN, scheduling and admin
- Participate in marketing and promotional events

About you...
We are looking for caring, kind and experienced Care Partner who is passionate about making a difference in lives of elderly.
To succeed in this role you will need the following qualifications, skills and attributes:
- Enrolled Nurse or Certificate IV or Diploma in Community/ Aged care / Disability
- Minimum 1.5 years previous experience in a similar role
- Great communication and interpersonal skills
- Ability to prioritise tasks, function independently as well as collaboratively with the team
- Excellent time management, organisational and quick decision-making skills.
- Full Australian working rights
- National Police Certificate
- First Aid and CPR certificates
- Have a reliable and insured vehicle
- Unrestricted NSW Driver License
