We are specifically looking for someone reliable, consistent, and genuinely interested in building a positive long-term support relationship with a participant.
If you frequently change jobs, struggle with communication, disappear during onboarding, or cannot commit to attending the same ongoing shifts consistently, this role will not be suitable.
National Disability Care (NDC) is a growing NDIS provider focused on delivering structured, person-centred, high-quality support. We are currently seeking a calm, patient and dependable Support & Outreach Coordinator to support a female participant in the Dandenong North area.
About the Participant
The participant is a 46-year-old female living with psychosocial disability and multiple chronic health conditions, including POTS, Heart Failure, Long COVID, Chronic Fatigue, Chronic Pain Syndrome and Asthma.
Her health and energy levels can fluctuate significantly from day to day. She benefits from support workers who are calm, emotionally regulated, flexible and able to work at a slower pace without pressure.
The participant experiences anxiety surrounding illness and infection exposure due to the impact COVID-19 has had on her health. Workers should approach these concerns respectfully and provide support that feels safe, collaborative and reassuring.
Trust, consistency and rapport are extremely important in this role.

This participant benefits from workers who understand that support is not always about keeping busy. Some shifts may focus on pacing, organisation, meal preparation, emotional reassurance and helping the participant manage daily life at her own pace.
Roster Available
- Every second Tuesday: 12:00pm – 5:00pm
- Weekly Saturday: 12:00pm – 6:00pm
Additional hours may become available in the future.

Mandatory Requirements
- Current Driver Licence
- Own reliable vehicle
- Comprehensive Car Insurance
- Willingness and ability to transport participants in your own vehicle
- NDIS Worker Screening Check
- Working With Children Check
- NDIS Worker Orientation Module
- National Police Check
- Current First Aid & CPR Certification

Experience
- Previous disability support experience preferred
- Experience supporting psychosocial disability highly regarded
- Understanding of chronic illness, fatigue management or mental health support highly regarded
- Experience with community access and appointment support desirable
- Strong communication and emotional regulation skills essential
