MAIN DUTIES/RESPONSIBILITIES
· Provide information required to be added in to support plans/profiles
· Develop and maintain positive relationships with Participants and both Internal and External stakeholders
· Assists NDIS Participants to identify and plan strategies to achieve their personal goals within a person-centered and holistic framework
· Works equitably with Participants with respect to each individual’s right to dignity, privacy, independence and respect
· Support Participants to implement their NDIS plan to maximise the value for money they receive from their supports, in line with reasonable and necessary criteria whilst maintaining appropriateness of the clients requirements and goals
· With the Participant’s approval and knowledge, provide information to Participants and advocates when necessary to facilitate access to community services and facilities
· Supports Participants in facilitating their independence by enabling empowerment and choice, and the promotion of personal resilience and social inclusion
· Networking to raise awareness of the NDIS and Participant’s disability requirements with mainstream supports
· Liaises with carers, families, guardians and community involved in providing care, including specialist generic agencies and other mainstream service provider
· Works collaboratively with other Support Coordinators within the Team and the Support Coordinator Officer
· Ethically and with integrity, operate within relevant legislation, and NDIA and NDIS policies and guidelines
· Ethically and with integrity, operate within relevant Diverse Learning policies and procedures
· Commitment to relevant legislative, NDIA, NDIS and Diverse Learning policies, guidelines and procedures changes, as they happen.
· Liaises regularly with the Support Coordinator Officer and operates collaboratively with, and within, the Support Coordination and Staff Team.
· Carries out administrative tasks, including entering accurate case notes in a timely manner and maintaining files and client contact and NDIS information as required
· Maintaining current Service Agreements and in-home risk assessments
· Provide accurate reports in adequate time frames
· Compliance with Diverse Learning’s values, code of conduct, policies and procedures and the relevant government legislation and standards
· Ensure adequate use of resource and funding
· Work collaboratively with external mainstream and community supports for professional support and opportunities to build and maintain capacity
· Promote a positive safety culture by contributing to health ,safety and welfare consultation and communication
· Promptly respond to and report health, safety and welfare hazards, incidents and near misses to the Work Health Safety Officer
SKILLS & EXPERIENCE
Qualifications:
· A tertiary qualification in Social work, Psychology Occupational therapy or equivalent OR 6 months experience working in a Support Coordination role
· A Current working with Children Check
· NDIS workers clearance
· RRHAN training or Child Safe Environments Training
· First Aid
· CPR
· Car Insurance, Driver’s license and a safe vehicle to provide transport.
Experience:
· At least 12 months working within the disability sector.
Skills:
· Demonstrated ability to work with people with a broad range of disabilities.
· Understanding and ability to work within the NDIS Code of conduct
· Leadership skills
· understanding of restrictive practices
· Ability to ensure positive outcomes for people with disabilities
· A person centered approach in all supports
· Ability to communicate with external stake holders
· Proven written and verbal communication skills
· Ability to be flexible in communication styles to suit a broad range of people
· Report writing capabilities

PERFORMANCE GOALS
· Person centered outcomes in appropriate time frames
· Ensure KPI’s are set with your direct manager, and that you’re working within those, or are able to provide an understanding where KPI’s are unable to be met
· Ability to attract and retain NDIS Support Coordination Participant
· Achievement of NDIS Participants’ goals moving towards independence
· Customer Satisfaction measures
· Other key measures developed by Diverse Learning, the NDIA, NDIS or other legislative requirements and key stakeholders.
Pay: $61,577.77 – $105,235.50 per year
