About the Role
We are seeking a motivated and compassionate Support Coordinator to join our team. This role involves supporting NDIS participants to understand and implement their plans, connect with providers, build capacity, and achieve their goals.
This role is primarily work-from-home with flexible arrangements, while also requiring some face-to-face participant meetings, stakeholder appointments, and community-based support across the Cairns and Tablelands regions.
Key Responsibilities
- Support participants to understand and implement their NDIS plans
- Connect participants with appropriate supports and services
- Coordinate appointments, stakeholder meetings, and referrals
- Maintain accurate case notes and participant records
- Monitor budgets and support utilisation
- Advocate for participants where required
- Liaise with families, guardians, providers, allied health professionals, and the NDIA
- Assist participants during plan reviews and change of circumstances requests
Essential Requirements
- Experience in the disability, mental health, community, or aged care sector
- Strong communication and organisational skills
- Ability to work independently and manage time effectively
- Current Driver’s Licence and reliable vehicle
- NDIS Worker Screening Check (Yellow Card) or willingness to obtain
- Current Blue Card
- Competent computer skills
Advantage:
- Previous NDIS Support Coordination experience
- Knowledge of the NDIS and PACE system
- Experience working with complex participants
- Certificate IV or higher in Community Services, Disability, Mental Health, or similar
