What you'll be doing
The Sustaining NSW Families Program (SNF) Child and Family Health Nurse (CFHN) practices at an advanced level of nursing, implementing a sustained health home visiting program which commences in pregnancy and continues to the child's second birthday.
The nurse works in a primary healthcare model with families who have been identified through the Safestart process. The SNF Nurse will perform health and development screening of infants/children in accordance with the Personal Health Record PHR schedule.
The SNF Nurse also completes assessments as per the SNF program home visiting schedule using screening tools. The nurse works as part of a multidisciplinary team utilising case reviews to plan care. The SNF Nurse promotes, encourages and supports breastfeeding and the maintenance of lactation. A family partnership and anticipatory guidance approach is utilised to promote optimal infant and maternal mental health, child development and positive parent child relationships.
About You
You describe yourself as a practitioner who brings depth, credibility and steadiness to Child and Family Health Nursing. You hold current authority to practise as a Registered Nurse or Midwife with a post‑graduate Child and Family Health Certificate through AHPRA, and you carry that qualification into your work with a strong sense of responsibility. With more than three years of post‑graduate experience in Child and Family Health Nursing, you’ve developed extensive knowledge in supporting families with children aged 0–2, and you draw on that experience to guide your clinical decisions and your interactions with parents and caregivers.
You understand the principles and purpose of the Sustaining NSW Families Program, and you can clearly articulate how your background aligns with its goals. Your practice reflects a commitment to early intervention, family‑centred care and evidence‑based support, and you’re able to translate that understanding into meaningful, practical work with vulnerable families.
You’re confident providing clinical consultation and facilitating case reviews, especially within multidisciplinary, community‑based teams. You know how to hold the family at the centre of the work while coordinating input from multiple professionals, ensuring that care remains cohesive, respectful and effective. Your communication skills strengthen your practice — you write clearly, speak with purpose and adapt your style to suit the needs of colleagues, clients and partner agencies. You’re comfortable using electronic medical records, word processing tools and databases, and you navigate digital systems with ease.
You build cooperative, professional and therapeutic relationships by drawing on strong interpersonal and conflict‑resolution skills. Whether you’re working with families, government services or NGO partners, you maintain a calm, collaborative presence that supports trust and shared problem‑solving. You’ve also led quality improvement initiatives that have driven clinical change, and you have experience planning, coordinating and evaluating education programs designed to improve outcomes for families.
You hold a current NSW driver’s licence and you’re willing to travel as needed, approaching the mobility of the role as part of your commitment to meeting families where they are and delivering care that is accessible, responsive and grounded in best practice.
