About the role:
As a Senior Personal Carer you'll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centered approach while providing quality care based on clients' needs.

Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.

Management of client information in CRM & CMS ensuring all details and case notes are kept up to date

Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.

Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities

Providing care in accordance with their individualised Care Plan

Supporting with community based recreational and social activities

Transportation and assistance with shopping and appointments.

Role requirements:

Certificate III Aged Care, Individual Support or an equivalent qualification - Essential

Current Australian Driver's Licence (Green Provisional minimum) - Essential

Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential

Experience with technology incl. smartphones, tablets, laptops etc

Confidence to satisfy the relevant probity checks required by legislation or policies
