The Role
Working across Residential Aged Care Homes in Launceston and surrounding areas, you will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents. You will work within a collaborative multidisciplinary model alongside on-site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.
What You'll Bring

Experience in aged care, gerontology, palliative care or chronic disease management is highly regarded

Advanced clinical experience in assessment, diagnosis, treatment, prescribing and health promotion

Strong clinical reasoning, communication and leadership capabilities

Experience in quality improvement and evidence-based practice

Sound knowledge of aged care legislation and regulatory requirements

Current driver's licence, reliable vehicle and appropriate professional indemnity insurance

Comfortable using technology and clinical software systems
Qualifications

Master of Nursing (Nurse Practitioner)

Current AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
