We are looking for an experienced and dedicated Clinical Manager to lead our clinical services and ensure the delivery of safe, evidence-based, and compliant care across our Home Support programs.
About the Role
As the Clinical Manager, you will oversee the delivery of clinical care, support clinical governance, ensure compliance with the Strengthened Aged Care Quality Standards, and drive continuous quality improvement. You will work collaboratively with nursing staff, Home Support Workers, Care Coordinators, General Practitioners, Allied Health Professionals, clients, families, and community partners to achieve positive client outcomes.
Key Responsibilities1. Clinical Governance & Compliance
- Ensure compliance with the Strengthened Aged Care Quality Standards, AHPRA requirements, organisational policies, and relevant legislation.
- Monitor clinical quality, documentation, incident reporting (including SIRS), and continuous quality improvement activities.
2. Clinical Care & Care Coordination
- Oversee clinical assessments, care planning, and the delivery of safe, person-centred care.
- Coordinate care with clients, families, General Practitioners, allied health professionals, hospitals, and community providers to achieve positive health outcomes.
3. Client Safety & Quality Care
- Promote safe home environments through effective risk management, medication safety, Infection Prevention & Control (IPC), and Antimicrobial Stewardship (AMS).
- Support clients' dignity, independence, choice, and wellbeing.
4. Staff Support & Clinical Development
- Provide clinical guidance, mentoring, and competency development for nursing and care staff.
- Promote evidence-based practice, continuous learning, and high standards of clinical care.
5. Quality Improvement & Stakeholder Engagement
- Participate in audits, accreditation, and quality improvement initiatives.
- Foster collaborative relationships with clients, families, healthcare professionals, and community partners to support high-quality home support services.
What We're Looking For
- Strong clinical assessment, care planning, and care coordination skills.
- Sound knowledge of clinical governance, aged care legislation, and the Strengthened Aged Care Quality Standards.
- Experience in medication management, clinical risk management, quality improvement, Infection Prevention & Control (IPC), and Antimicrobial Stewardship (AMS).
- Excellent leadership, communication, coaching, and relationship-building skills.
- Strong organisational, problem-solving, and decision-making abilities.
- A commitment to delivering safe, evidence-based, and person-centred care.
Essential Qualifications & Experience
- Bachelor of Nursing with current unrestricted AHPRA registration as a Registered Nurse.
- Minimum 3 years' experience in aged care, home care, community nursing, or a related healthcare setting.
- Previous experience as a Clinical Manager, Care Manager, Clinical Care Coordinator, or in a senior nursing role.
- Current First Aid & CPR Certificate.
- Medication competency and Infection Prevention & Control (IPC) training.
- Current National Police Check.
Desirable
- Postgraduate qualification in Aged Care, Clinical Leadership, or Health Management.
- Additional training or certification in Dementia Care, Palliative Care, or Wound Care.
- Experience in quality improvement, clinical auditing, or accreditation.
