About the Role
This role provides an opportunity to support Aboriginal patients and families receiving palliative and end-of-life care across the Mid North Coast. As an Aboriginal Health Practitioner, you will deliver culturally safe clinical care, advocacy, and support while working closely with patients, families, community organisations, and multidisciplinary health teams.
You will play an important role in ensuring care planning respects Aboriginal cultural practices and perspectives around death and dying. The role includes contributing to care coordination, supporting Palliative Care Outcome Collaboration (PCOC) assessments, and building strong partnerships with Aboriginal communities to ensure services are culturally responsive. Travel across the Mid North Coast will be required to support patients and families in rural and remote locations.
About You
You are an Aboriginal and/or Torres Strait Islander Health Practitioner with a strong commitment to improving health outcomes for Aboriginal communities. You hold a Certificate IV in Aboriginal Primary Health Care Practice and are registered with AHPRA as an Aboriginal Health Practitioner.
You bring experience or a strong interest in palliative and end-of-life care and understand the importance of culturally safe support for Aboriginal patients and families. You are confident working with communities, families, and multidisciplinary teams to provide coordinated, respectful care.
You have strong communication skills, experience using electronic medical records, and hold a current driver’s licence with the ability to travel across the Mid North Coast region.
