We are seeking a compassionate, professional and experienced Registered Nurse (RN) or Enrolled Nurse (EN) to join our busy and growing General Practice team.
The successful candidate will have a strong understanding of general practice nursing, excellent clinical and communication skills, and a genuine interest in providing high-quality patient-centred care.
The role will primarily be based within the medical practice, with the opportunity and requirement to travel with our General Practitioners to aged care facilities to assist with consultations, clinical assessments, chronic disease management and ongoing patient care.

Key Duties & Responsibilities
The Practice Nurse will work closely with General Practitioners, Practice Management and the wider clinical team to provide safe, efficient and patient-centred nursing care across the general practice and aged care settings.
General Practice Nursing
- Provide high-quality nursing care to patients presenting to the general practice.
- Conduct clinical assessments, including vital signs, observations, health assessments and relevant screening.
- Assist GPs with consultations, procedures, examinations and minor surgical procedures.
- Perform wound care, dressings, suture and staple removal and other nursing procedures within scope of practice.
- Administer vaccinations and medications in accordance with relevant legislation, clinical protocols and practice policies.
- Assist with immunisation programs, including childhood, adult and seasonal vaccination clinics.
- Undertake health assessments, preventive health checks and chronic disease management activities.
- Support GPs with care plans, referrals, follow-up appointments and ongoing patient monitoring.
- Assist with the management of patients with chronic and complex health conditions.
- Perform ECGs, spirometry, pathology collection and other clinical procedures as appropriately trained and within scope of practice.
- Provide patient education regarding medications, lifestyle, preventative health, chronic disease management and self-care.
- Assist with recalls and reminders, including immunisations, chronic disease reviews, screening and preventive health activities.
- Maintain accurate, timely and comprehensive clinical documentation and health records.
- Ensure all nursing activities comply with relevant legislation, professional standards, infection prevention and control requirements and practice policies.
Aged Care Facility Responsibilities
- Travel with the GP to nominated aged care facilities as required.
- Assist the GP during on-site consultations and clinical reviews of residents.
- Undertake observations, clinical assessments and nursing assessments as requested by the GP.
- Assist with chronic disease management and monitoring of residents with complex health needs.
- Support medication reviews, wound management, continence care, pain assessment and other nursing interventions within scope of practice.
- Assist with care planning, clinical reviews and follow-up of residents.
- Liaise with aged care facility staff regarding residents' clinical needs and communicate relevant information to the GP.
- Monitor and escalate changes in a resident's condition promptly and appropriately.
- Assist with coordination of pathology, referrals, investigations and follow-up care where required.
- Support the GP in providing comprehensive, person-centred care that respects the dignity, preferences and individual needs of each resident.
- Maintain appropriate clinical documentation following aged care consultations and ensure information is accurately recorded in the relevant medical record.
Clinical Support & Patient Care
- Provide compassionate, respectful and culturally appropriate care to patients of all ages.
- Recognise and appropriately escalate deteriorating or high-risk patients.
- Assist with triage and prioritisation of patient care within the nurse's scope of practice.
- Provide telephone and in-person clinical support to patients where appropriate.
- Promote preventive health and early intervention.
- Develop positive relationships with patients, families, carers, GPs and other healthcare professionals.
- Maintain patient confidentiality and comply with privacy and health information requirements.
Practice Operations & Quality
- Maintain a high standard of infection prevention and control, including appropriate cleaning, sterilisation and clinical waste management practices.
- Ensure clinical rooms are appropriately stocked, clean, safe and prepared for daily operations.
- Monitor and maintain vaccine cold-chain requirements and complete relevant documentation.
- Check emergency equipment, clinical supplies and medications as required.
- Participate in clinical audits, quality improvement activities and accreditation requirements.
- Follow practice policies, procedures, clinical protocols and work health and safety requirements.
- Participate in staff meetings, training, professional development and competency assessments.
- Work collaboratively with reception, administration, GPs, allied health professionals and practice management to support efficient patient flow.

Essential Skills & Attributes
The successful applicant will demonstrate:
- Current RN or EN registration with AHPRA.
- Previous experience in general practice, primary care, community nursing or aged care is highly desirable.
- Strong clinical assessment and communication skills.
- Ability to work independently while collaborating effectively with GPs and the broader clinical team.
- Confidence working with elderly patients and people with chronic and complex health conditions.
- Excellent organisational and time-management skills.
- Strong attention to clinical documentation and detail.
- A compassionate, respectful and patient-centred approach.
- Ability to travel to aged care facilities with GPs as part of the role.
- Current immunisation and relevant clinical competencies as required.

Pay: $30.00 – $40.00 per hour
