About the role:
A typical week is varied and includes travel across multiple sites, working as part of a dedicated and collaborative multidisciplinary team of nursing and allied health professionals. The role is centred on delivering culturally safe, person-centred chronic disease care that supports Aboriginal people, their families, and communities, while fostering respectful partnerships and improving health outcomes.
Where you’ll be working:
An exciting opportunity exists for an Aboriginal Health Practitioner/Worker to join the Integrated Chronic Care for Aboriginal People Program, based at Inverell Community Health.
In this role, you'll work in partnership with a Clinical Nurse Consultant and a supportive multidisciplinary team to provide culturally safe, holistic care for Aboriginal people living with chronic and complex health conditions, including diabetes, cardiovascular disease, and respiratory illness. You'll play an important role in delivering health assessments, care coordination, health education, and ongoing support that empowers individuals, families, and communities to achieve better health outcomes.
Based in Inverell, this temporary part-time position also provides outreach services across Ashford, Tingha, Glen Innes, Tenterfield, Armidale, Uralla, and Guyra, offering the opportunity to build meaningful relationships with Aboriginal communities throughout the region.
We're looking for a motivated, compassionate, and community-focused Aboriginal Health Practitioner/Worker who is committed to improving Aboriginal health outcomes and enjoys working collaboratively within a multidisciplinary team.
About you
Experience working with Aboriginal people living with chronic disease and providing health education and support will be highly regarded. Knowledge of diabetes, cardiovascular and respiratory care can be developed in the role, with ongoing support from the Integrated Chronic Care Clinical Nurse Consultant, your line manager, and the broader Integrated Chronic Care for Aboriginal People Program team.
Requirements
For an Aboringinal Health Worker: Holds or aspires to hold a Cert III qualification in Aboriginal and Torres Strait Islander Primary Health Care and or Certificate III in other health qualification relevant to the position.
For an Aboringinal Health Practitioner: Holds or aspires to hold a Cert IV qualification in Aboriginal and Torres Strait Islander Primary Health Care and or Certificate IV in other health qualification relevant to the position.
