As part of our multidisciplinary Carers Independent Living Support team, you’ll deliver flexible, person-centred support across a range of programs including HACC and carer support services, alongside other community-based initiatives.
You’ll work with people from diverse backgrounds, helping them navigate changing needs, stay connected, and build independence. This is a dynamic role that combines early intervention, practical support, and community engagement, all with a strong focus on inclusion and wellbeing.
What you will be doing:
- Building strong, trusting relationships with carers, clients, and families.
- Providing assessment, goal planning, and short-term targeted support.
- Coaching and connecting people with specialist services and community programs.
- Supporting group activities and community participation.
- Delivering practical assistance that promotes independence in daily life.
- Strengthening caring relationships and reducing social isolation.
- working across multiple programs to provide coordinated, holistic support.

A relevant qualification in Social Work, Community Services, Psychology, Counselling, OR a related discipline, OR demonstrated equivalent knowledge and experience relevant to community services practice.
