At B Able Support Coordination, we believe in choice, control, and empowering individuals. We work closely with people experiencing permanent and significant disabilities to help them navigate their NDIS plans, overcome barriers, and achieve their personal goals. We are looking for passionate, driven, and knowledgeable Support Coordinators and Psychosocial Recovery Coaches to join our growing team and make a genuine difference in our communities.

NDIS Support Coordinator

The Support Coordinator is an administrative and strategic logistical role funded by the National Disability Insurance Scheme (NDIS). Their primary job is to help participants understand, implement, and maximise the funding in their NDIS plans.

Core Focus: Plan implementation, system navigation, and capacity building.
Key Responsibilities:
Connecting participants with mainstream, community, and NDIS-funded service providers (e.g., allied health, support workers, cleaner, housing).
Setting up service agreements and monitoring budgets so funds don’t run out early.
Troubleshooting when a service provider isn't working out.
Preparing evidence and writing comprehensive reports for the participant’s NDIS plan review.

NDIS Psychosocial Recovery Coach

A Psychosocial Recovery Coach is also funded by the NDIS, but it is a specialised, relational coaching role specifically for participants whose primary disability stems from a severe mental health condition (a psychosocial disability).

Core Focus: Mental health recovery, resilience-building, and day-to-day motivation.
Key Responsibilities:
Providing hands-on, one-on-one coaching to build self-advocacy, daily routines, and coping strategies.
Collaborating with the participant, their family, and clinical teams to design a personalised Recovery Plan.
Helping the participant navigate both the NDIS and the broader state-funded mental health system.
Using a strengths-based approach to foster independence and purpose.

NDIS Clinical Care Coordinator

It is an active clinical leadership role designed to oversee a participant's clinical needs are being meet, and they have the support they need for optimal wellness; to ensure key providers are all following a rehabilitative treatment or clinical care need support plan.

Core Focus: Clinical and wellness care plans, medication management oversight, and multidisciplinary health coordination.
Key Responsibilities:
Conducting formal clinical and risk assessment profiles.
Monitoring, and requesting reviews when necessary from medical/nursing care plans (e.g., mitigating risk for complex chronic diseases, wound care, medication regimes, or palliative care).
Triaging referrals and coordinating communication between GPs, medical specialists, hospital discharge teams, and allied health professionals.
Ensuring compliance with healthcare accreditation and safety standards.
Qualifications: Strictly clinical. This role requires current registration as a health professional—most commonly a Registered Nurse (RN), but occasionally a qualified Occupational Therapist, Social Worker, or Psychologist with extensive clinical experience.

As a Coordinator, you will be the bridge between our participants and the life they want to live. You will establish positive, collaborative relationships with participants and their support networks, helping them identify and link with local community and mainstream services.

You will solve basic to more complex problems to mitigate risks and barriers in participants' lives, and build capacity to allow autonomy and positive plan implementation that aligns with the NDIS safeguards.

Key Responsibilities

Empower Participants: Support individuals to coordinate their NDIS plans, build their capacity, and negotiate services that align with their goals.
Build Connections: Identify and link participants with local community, mainstream, and specialist support networks.
Plan Implementation: Assistance to allow the smooth rollout of participant plans and track progress toward their individual goals.
Administration & Compliance: Complete support coordination within agreed utilisation schedules and accurately document/lodge reports to the NDIA and the Safeguarding Commission.
Create person centred practice: upskill in person centred training, and communicate with participants in line with their communication style and needs.

What We Are Looking For

NDIS Expertise: A strong understanding of the NDIS framework, price guide, and the distinct role of a Support Coordinator.
The Right Attitude: A positive, collaborative approach with excellent communication and relationship-building skills.
Organisation Skills: Ability to manage a caseload, maintain accurate case notes, and navigate the NDIA portal.
Qualifications: Cert IV/Diploma in Community Services, Disability, Mental Health, Clinical Care or equivalent experience (beneficial but not necessary).
Resilience: Ability to work in a fast-paced and complex role that provides truly life-saving support to Australians living with an impairment.
Must-Haves: Current NDIS Worker Screening Check, Working with Children Check, Criminal History Record Check, and a valid Driver’s License with a reliable vehicle (obtained prior to employment).
