About the Role
As a Clinical Nurse, you will deliver safe, evidence‑based clinical care aligned with the Aged Care Quality Standards, NSW Health requirements and Council policies. You will play a key role in clinical governance, risk management and continuous improvement across Council’s aged and community care programs.
Key Responsibilities
- Deliver clinical assessments, care plans, wound care, medication support and chronic disease monitoring
- Identify and escalate clinical deterioration; maintain accurate clinical documentation
- Work collaboratively with Participants, families, carers and multidisciplinary teams
- Promote wellbeing, independence and culturally safe, person‑centred care
- Manage clinical risks, incidents, infection control and safe work practices
- Provide clinical guidance, mentoring and training to support workers and new staff
- Liaise with GPs, allied health, hospitals and external providers to coordinate care
- Maintain accurate records, prepare reports and contribute to service planning and improvement
About You
Essential
- Current AHPRA registration (Registered Nurse – Division 1)
- Clinical experience in aged care, community nursing or chronic disease management
- Strong assessment and care‑planning capability
- Understanding of Aged Care Quality Standards and clinical governance
- High‑level communication, documentation and digital literacy
- Ability to work collaboratively with Participants, families and multidisciplinary teams
- Current NSW Driver Licence, National Police Check and WWCC
Desirable
- Postgraduate qualifications in Aged Care, Community Health or Chronic Disease
- Experience in local government or community‑based service delivery
- Knowledge of My Aged Care and the Support at Home Program
- Experience delivering staff or clinical training
