DISABILITY SUPPORT WORKER – FULL-TIME

Employer: NASHE GROUP PTY LTD
Location: [Geraldton WA,6530]
Employment type: Full-time, ongoing
Hours: 38 hours per week, including rostered shifts
Salary: AUD $[76,000]–$[85,000] per annum plus superannuation

About the role

We are seeking an experienced and compassionate Disability Support Worker to provide high-quality support to participants in their homes and within the community.

The successful applicant will work collaboratively with participants, families and other support professionals to implement individual support plans and help participants achieve their personal goals.

Key responsibilities

Provide person-centred support in accordance with individual support and care plans.
Assist participants with personal care, mobility, meal preparation, household activities and daily living tasks.
Support participants to develop independent living and social skills.
Facilitate community access, appointments, recreational activities and social participation.
Provide medication assistance within the employee’s training and authorised scope.
Follow positive behaviour support plans and use appropriate de-escalation techniques.
Identify and report changes in participants’ health, wellbeing or support requirements.
Maintain accurate progress notes, incident reports and other service records.
Communicate professionally with participants, families, support coordinators and colleagues.
Follow safeguarding, privacy, infection-control, workplace health and safety, and organisational requirements.
Work rostered shifts, which may include evenings, weekends, public holidays and sleepover shifts.

Essential qualifications and experience

Certificate III or IV in Individual Support (Disability), Disability Support, Community Services or another relevant qualification.
At least 12 months of relevant paid experience supporting people with disability.
Demonstrated understanding of person-centred practice, participant choice and dignity of risk.
Strong communication, observation, documentation and interpersonal skills.
Ability to work independently and as part of a multidisciplinary support team.
Current First Aid and CPR certificates, or ability to obtain them before commencing.
Current NDIS Worker Screening Check, or ability to obtain one.
National Police Check and Working with Children Check where required.
Current unrestricted Australian driver’s licence and [access to an insured vehicle, if required].
Ability to meet the physical and inherent requirements of the position.

Desirable experience

Supporting people with complex needs, psychosocial disability or behaviours of concern.
Positive behaviour support and de-escalation.
Medication assistance, manual handling or complex personal care.
Experience preparing compliant progress notes and incident reports.
