About the role

You will work closely with clients, carers and interdisciplinary teams to deliver evidence-based occupational therapy services that improve wellbeing, independence and quality of life. The client population is primarily older adults, many of whom have chronic and complex presentations, however, the program caters for all adults.

Your days could include:

Conducting comprehensive OT assessments and develop client-centred care plans

Completing home visits and recommend assistive technology, equipment and home modifications

Working with funding through My Aged Care, CHSP, HACC PYP, Support at Home, SWEP and Geat2Go

Providing coaching, education and self-management support to clients and carers

Collaborating with Physiotherapists, Podiatrists, Allied Health Assistants and other clinicians

Participating in supervision, case reviews and quality improvement initiatives

Maintaining accurate documentation and contribute to service KPIs

About you

You are a compassionate and motivated Occupational Therapist who values teamwork, continuous learning and community health.

You will bring:

Current AHPRA registration (or eligibility)

Experience or knowledge in functional assessments, home modifications and assistive technology

Strong communication, organisation and problem-solving skills

Ability to work effectively with diverse and CALD communities

Commitment to evidence-based practice and high-quality client care
