Based across multiple South-East Melbourne locations, including South Melbourne and St Kilda, you'll enjoy a balance of independence, collaboration and meaningful client relationships.
About the Role
As an Occupational Therapist Grade 2, you'll work with adults living with chronic and complex health conditions to maximise their independence, participation and quality of life.
You'll partner with clients, carers and other health professionals to deliver evidence-based, person-centred care in home and community settings.
You will:
- Conduct comprehensive occupational therapy assessments and develop client-centred care plans
- Complete home visits and provide recommendations for assistive technology, equipment and home modifications
- Support clients navigating My Aged Care, CHSP, HACC PYP, Support at Home, SWEP and other funding pathways
- Deliver education, coaching and self-management support
- Collaborate closely with Physiotherapists, Podiatrists, Allied Health Assistants and other clinicians
- Participate in supervision, case reviews and continuous improvement initiatives
- Maintain quality documentation and contribute to service outcomes and KPI achievement
About You
You're a motivated Occupational Therapist who enjoys building relationships, working collaboratively and helping people achieve what matters most to them.
You'll bring:
- Current AHPRA registration (or eligibility for registration)
- Experience or knowledge in functional assessments, home modifications and assistive technology
- Strong communication, organisation and problem-solving skills
- The ability to work effectively with people from diverse cultural and linguistic backgrounds
- A commitment to evidence-based practice and high-quality client care
