About You
Do you hold formal qualifications in the community sector – Social Work, Psychology, Cert V in Mental Health or Mental Health Peer Work or other relevant studies? Do you have a strong understanding of community mental health and services in the South East of Melbourne, and experience in case management? Do you have the passion to use your experience in supporting individuals requiring psychosocial support to create linkages within their community and build their capacity and aide in their recovery?

Ability to work both autonomously, and contribute positively within a team, sharing ideas and working collaboratively
Effective listening skills & an adaptable communication style

Lived or living experience of mental ill-health, or as a carer for someone living with dual diagnosis/disability are encouraged to apply
Qualifications in Intentional Peer Support or a willingness to undertake training

We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes, including (but not limited to) wearing appropriate PPE and regular hygiene and cleanliness practices.

About the Role
The Wellbeing Mentor Lived or Non-Lived Experience role provides assertive community outreach and coordination of service provision for people with significant mental illness funded through the Commonwealth Psychosocial Support (CPS) program. The goal is to promote independence and build confidence of individuals. As a Wellbeing Mentor, there will be opportunities for you to assist with working alongside participants, providing strengths-based, trauma-informed and recovery-oriented psychosocial rehabilitation services through your lived experience lens.

The role entails:
Providing case management and assertive outreach to support a portfolio of participants and their families in their own home and community
Providing short term assistance to enable participants to work towards their goals
Assisting to build capacity and working towards outcomes identified by the participant
Developing and maintaining professional relationships with participants and stakeholders
Providing accurate and timely case notes including risk management and recovery plans
This role involves significant outreach covering a broad catchment area of over 10 LGAs in the South East region of Melbourne. Use of a personal vehicle will be required.
Hybrid working model with flexible service delivery between 8am-8pm Monday-Friday based on consumer needs.

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver’s licence.
