About the Role

Juniper is seeking a culturally responsive and community-focused Customer Care Lead to join our Home and Community Care (HCC) team in the North metropolitan area of Perth. In this vital role, you’ll be the trusted connection for our customers and their families, building strong relationships, coordinating services, and ensuring care is delivered with respect, empathy, and cultural understanding.

You’ll oversee a portfolio of government-funded programs including Support at Home (SAH), Commonwealth Home Support Program (CHSP). Your leadership will ensure services meet the highest standards of care and compliance under the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards.

We are seeking an experienced Care Partner to join the team on a fixed term basis until April 2027. The successful candidate must be able to work 0830-1630 Monday to Friday.

Key Responsibilities:

Manage and coordinate community-based aged care programs with a focus on customer goals and wellbeing
Lead and support Community Support Workers through recruitment, onboarding, supervision, and development
Monitor client health and wellbeing, ensuring early intervention and appropriate referrals
Deliver trauma-aware, healing-informed care that respects each individual’s story
Ensure compliance with legislation, funding guidelines, and Juniper’s policies
Promote a safe and healthy work environment through proactive WHS practices

What You’ll Bring:

Experience in case management or coordination within aged care, disability, or community services
Strong understanding of aged care funding programs and person-centered care principles
Excellent communication and leadership skills
Ability to manage budgets, develop care plans, and support staff performance
Relevant qualifications in community services, aged care, or health (minimum Certificate IV)
Current driver’s license and access to a reliable vehicle
National Police Clearance

Hourly rate of $39.60 per hour + 12 % superannuation
Salary Package up to $18,550 per annum tax free
