About the Opportunity
Join the Care Coordination Team within Community Mental Health and provide clinical leadership that supports high-quality, coordinated care for consumers experiencing moderate to severe mental illness. Working across community health centres on the Central Coast, you will play a key role in supporting clinical decision-making, consumer flow and effective multidisciplinary practice.
In this role you will:

Provide clinical leadership and guidance to the multidisciplinary Care Coordination Team, supporting high-quality clinical care and consumer safety.

Coordinate and triage referrals, supporting timely assessment, allocation and progression of consumers through the service.

Monitor clinical governance requirements, including clinical documentation, mandatory clinical processes, risk management and clinical notifications.

Collaborate with mental health teams, consumers, families, carers and internal and external service providers to support seamless, consumer-centred care.

Contribute to service development, quality improvement and organisational change while supporting a positive workplace culture and the ongoing professional development of staff.

About You
We are looking for an experienced mental health clinician who is passionate about providing contemporary, consumer-centred care and can confidently lead clinical practice within a multidisciplinary community mental health environment.
We are looking for someone who has:

Extensive recent clinical experience in a community mental health setting, with the relevant qualifications and professional registration for the position.

Demonstrated experience providing clinical leadership, guidance and support to multidisciplinary teams and less experienced staff.

Strong knowledge of contemporary mental health practice, clinical governance, risk management, mandatory reporting requirements and relevant clinical tools.

Highly developed communication, collaboration and relationship-building skills, with the ability to work effectively with consumers, families, carers and internal and external service providers.

Demonstrated strategic leadership skills, including the ability to support organisational change, drive quality improvement and contribute to a positive and collaborative workplace culture.
