Role responsibilities
· Coordinate Client assessments, care planning, reviews, and evaluations
· Build trusted relationships with residents and families, monitoring wellbeing
· Explain care and funding options clearly and compassionately
· Connect Clients with health and support services as required
· Collaborate with GPs, Nurse Practitioners, allied health, and external providers
· Ensure accurate documentation and use of relevant systems
· Conduct regular client and family check-ins
· Complete assessments and participate in case conferences
· Experience in delivering clinical care
· Contribute to quality improvement, onboarding and positive team culture
Skills and Experience
· We’re looking for experienced Registered Nurses who:
· Hold current AHPRA registration
· Have a minimum 2 year clinical experience.
· Bring aged care or community experience (highly desirable)
· Have experience in, or a strong interest in developing, dementia care practice
· Are passionate about providing high-quality, person-centred care
· Feel confident using their clinical judgement and critical thinking
· Value taking time to build relationships with clients and families
· Are interested in stepping into a Case Manager/coordination role
· Want to make a real difference to how care is experienced
· Demonstrate a commitment to continuous professional development
. Casual initially, with the opportunity to transition to Part-Time.
Pay: $47.00 – $50.00 per hour
