About the Role
As a Lifestyle Facilitator (Support Worker), you will provide tailored supports that promote independence, community inclusion, and improved quality of life for people living with disability.
You'll work alongside Participants to build confidence, develop life skills, and achieve the goals outlined in their NDIS plans.
Your responsibilities will include:
- Supporting Participants with daily living activities and personal care
- Encouraging independence, choice, and decision-making
- Assisting with social, recreational, educational, and employment-related activities
- Facilitating meaningful community participation
- Implementing Positive Behaviour Support strategies where required
- Supporting Participants with complex needs in a respectful and person-centred manner
- Assisting with household tasks including meal preparation, cleaning, and laundry
- Performing safe manual handling tasks, including hoist transfers and wheelchair assistance
- Working collaboratively with families, allied health professionals, and other support networks
- Maintaining accurate documentation and complying with all abbaCare policies, NDIS Practice Standards, and legislative requirements

Essential Requirements
- Availability to work weekday morning shifts
- Demonstrated commitment to person-centred support
- Respect for diversity, inclusion, and cultural safety
- Compassionate, reliable, and professional approach
- Ability to advocate for Participants while promoting independence
- Excellent communication and interpersonal skills
- Experience supporting people with disability and/or mental health conditions
- Experience supporting Participants with complex behaviours and support needs
- Competency in manual handling techniques
- Experience working active overnight shifts
- Ability to work independently and as part of a multidisciplinary team
Mandatory Requirements
Applicants must hold or be willing to obtain:
- Queensland Green P-Plate or Open Driver Licence
- Reliable, fully comprehensively insured vehicle
- Current Blue Card (Working with Children Check)
- NDIS Worker Screening Clearance (Yellow Card)
- Completion of the NDIS Worker Orientation Module
- Current First Aid and CPR Certificate
- Evidence of COVID-19 vaccination (minimum two doses, where required)
Desirable Skills and Experience
While not essential, the following will be highly regarded:
- Experience working within Supported Independent Living (SIL)
- Knowledge of Positive Behaviour Support Plans
- Understanding of privacy, confidentiality, and safeguarding principles
- Experience building collaborative relationships with external service providers
