You will deliver a disciplined approach to the implementation of the Australian National Aged Care Classification (AN-ACC) to achieve Palm Lake Care financial KPIs by appropriately balancing care needs and resident choice. To be successful requires active listening and true leadership that others want to follow, you must be willing to travel to other communities and work within a broader team. You are a passionate and effective educator able to motivate and guide the behaviour of busy colleagues. In your role, you accomplish this task by applying comprehension supported by a steadfast dedication to achieving successful documentation results.
About Your Role
As the Clinical Coordinator - Admission & Assessments, you will be primarily responsible for foundational assessment and quality documentation of resident needs, and development of individual care plans. Also required will be the management and referral for ANACC reviews through My Aged Care, to ensure resident funding. You will communicate with clinical and care colleagues, management, and residents on an ongoing basis to ensure quality documentation outcomes.
The dignity and integrity of each resident’s independence will be based on your comprehensive and accurate nursing assessments, and your engagement in nursing practices, allowing you to understand and see the positive changes firsthand.
About Your Experience
Registered Nurse qualification with current AHPRA registration.
Demonstrated knowledge of ANACC, including assessments, claims submissions and working within policies and guidelines.
Proven knowledge of current trends in nursing and current clinical expertise in health and aged care.
Understanding of new Aged Care Quality Standards and the relationship with ANACC.
Demonstrated previous experience with funding business rules and assessment procedures, including ANACC validations.
Highly developed interpersonal communication and negotiation skills, particularly in the development and maintenance of relationships with older people, general practitioners, and other healthcare providers.
Ability to coordinate work, including the ability to establish priorities to meet deadlines and commitments.
Knowledge of Autumn Care would be highly regarded.
Experience in commissioning an aged care facility would be advantageous.
