Role Overview
The District Nurse Coordinator is responsible for the coordination, leadership, and day-to-day management of district nursing services within the community. This fixed-term role ensures continuity of high-quality, patient-centred care during a period of leave, supporting staff, managing service delivery, and maintaining clinical standards.
Key Responsibilities
Clinical Leadership & Coordination

Provide leadership and oversight to the district nursing team

Coordinate daily service delivery, including patient allocation and workload management

Ensure care is delivered in accordance with clinical guidelines, policies, and best practice standards

Support complex clinical decision-making and escalation as required
Staff Management & Support

Supervise, mentor, and support nursing and support staff
-
Assist with rostering, leave management, and workforce planning
-
Facilitate team communication and contribute to a positive workplace culture

Identify training and development needs and support ongoing education
Patient Care & Quality
-
Ensure delivery of safe, effective, and person-centred care in community settings

Monitor care plans and outcomes, ensuring appropriate documentation and follow-up

Manage patient flow, referrals, and service access

Address feedback, complaints, and clinical incidents in a timely manner
Operational Management

Oversee daily operations to ensure efficient service delivery

Liaise with internal departments and external stakeholders (e.g., GPs, allied health, hospitals)

Contribute to reporting, audits, and quality improvement initiatives

Maintain compliance with relevant healthcare standards and regulations
Essential

Registered Nurse Division 1, APHRA registration

Relevant Postgraduate qualification

Relevant experience in Community Nursing
