THE ROLE
We're looking for a Support Coordinator (Coordinator of Supports / CoS) to join our team serving NDIS participants across Greater Western Sydney and the Blue Mountains.
This is a Level 2 Support Coordination role with a genuine pathway toward Level 3 / Specialist Support Coordination as your experience and caseload grows. You'll be paid at the appropriate classification level under the Social, Community, Home Care and Disability Services Industry Award (SCHADS Award MA000100), based on your qualifications and experience.
A CoS at Jeder:
· Understands the NDIS deeply and helps participants do the same
· Builds confidence and capacity in participants and their support networks
· Is independent of service providers - your loyalty is to the participant
· Uses plan budgets creatively to implement goals, not just tick boxes
· Creates opportunities for participants to grow their independence over time
Day to day, this looks like:
· Helping participants understand and maximise their NDIS plans
· Connecting participants with services, community and informal supports that genuinely fit their life
· Facilitating complex, multi-provider supports where they intersect with mainstream services
· Crisis resolution and building participant resilience
· Monitoring plan budgets across a 12-month plan cycle
· Completing service agreements, referrals, risk assessments and NDIA reports
· Supporting participants through plan reviews - developing goals, evaluating supports, preparing documentation
· Regular outcome reporting to the NDIA
A CoS is not a paid advocate, a rostering service, an administrator, or a personal care worker. If that's the kind of support coordination you've been doing, this role will feel different.

ABOUT YOU
You'll thrive here if:
· You hold a Diploma or Certificate IV in Community Development, Community Services, Disability, or have equivalent relevant experience
· You have solid, practical knowledge of the NDIS and experience navigating it with and for participants
· You have experience engaging with inter-agencies and disability networks
· You have experience working with people with mental health and/or psychosocial conditions (Recovery Coaching background is a plus)
· You're a strong communicator - clear, warm and honest, especially when things are complicated
· You can self-organise, prioritise your own workflows and meet deadlines without someone managing your diary
· You're comfortable with complexity, adaptable and genuinely creative in how you problem-solve
· You're committed to a strengths-based, person-centred approach - not just as a phrase, but as a practice
· Training in Person-Centred Thinking and Planning, Asset Based Community Development (ABCD) or Art of Hosting is a bonus
You may struggle here if:
· You prefer a high-structure environment with close day-to-day supervision
· You're not yet confident working autonomously with complex or multi-provider cases
