The Registered Nurse (RN) plays a critical role in delivering direct patient care in a Community industry. RNs are responsible for assessing patients' conditions, implementing nursing care plans, administering medications, and collaborating with other healthcare professionals to ensure optimal patient outcomes. This role demands a combination of clinical knowledge, compassionate patient care, and effective communication skills.

Responsibilities:

Patient Assessment & Monitoring:
- Conduct thorough physical assessments of Clients.
- Monitor and document vital signs, symptoms, and changes in Clients conditions.
- Evaluate lab results, diagnostic tests, and clinical data.
- Visiting community and SIL houses in a regular basis.

Care Planning & Coordination:
- Develop, implement, and evaluate individualized care plans.
- Collaborate with physicians, other healthcare providers, and multidisciplinary teams.
- Educate patients and families about health conditions, treatment options, and preventive measures.

Medication Administration:
- Administer prescribed medications and treatments per physicians’ orders.
- Monitor patients for adverse reactions or side effects.
- Maintain accurate medication records and ensure proper handling of medications.

Clients' Advocacy & Communication:
- Serve as the primary advocate for patient needs, concerns, and rights.
- Communicate effectively with patients, families, and healthcare teams.
- Provide emotional support to patients and families.

Infection Control & Safety:
- Follow infection control protocols and maintain a clean, safe environment.
- Assist in the prevention of hospital-acquired infections.
- Ensure compliance with safety regulations and patient care standards.

Documentation & Record Keeping:
- Maintain accurate and up-to-date patient records and care documentation.
- Ensure compliance with legal, regulatory, and hospital standards regarding documentation.

Emergency & Critical Care:
- Provide emergency care in critical situations, including CPR and life-saving measures.
- Assess, stabilize, and monitor patients in emergency settings.

Skills Required:

Education:
- A valid Bachelor’s degree in Nursing or Associate Degree in Nursing (ADN).
- Must hold a valid and active Registered Nurse (RN) license in the state or region of employment.
Experience:
- Previous experience in Community nursing.
- Experience in specific specialties (e.g., ER, ICU, Pediatrics) may be required based on department needs.
Skills:
- Strong clinical assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively under pressure in fast-paced environments.
- Knowledge of medical equipment, clinical procedures, and patient care technologies.
- Compassionate, empathetic, and patient-centered approach.
