- Fixed term, full time role (12 month contract) – 80 hours per fortnight with a monthly ADO
- Parkville location, with flexible, hybrid working
- Up to $76,764 base + salary packaging + super + 5 weeks annual leave

About the team
Our Patient Navigation team plays an integral role in delivering compassionate, coordinated cancer care. Working in partnership with multidisciplinary clinical teams, we help ensure patients experience a seamless journey through the health service by providing administrative support, coordinating communication, and helping patients access the care they need. Our work makes a meaningful difference to both the patient experience and the delivery of high-quality care.
About the role
Our Patient Navigators are key members of the administrative team, working closely with clinicians to support patients throughout their care pathway.
In this role, you will:
- Be the first point of contact for patients, carers, and community referrers - responding to questions, resolving appointment issues, and helping to clarify instructions
- Coordinate communication between patients and their clinical care team, ensuring timely and clear information flow
- Facilitate patient access by managing patient flow and assisting with navigation through the cancer care system
- Work within a tumour stream model, across multiple tumour streams, allowing you to develop specialist knowledge and build strong, ongoing relationships with patients and their multidisciplinary care teams
This is a complex but rewarding and meaningful role that makes a valuable contribution to the care of cancer patients. You'll undertake a steep learning curve, but will be supported by a dedicated trainer through a comprehensive training program.

You will be someone who is resilient, a fast learner, calm under pressure, articulate, and able to communicate confidently and assertively when required.

Position Summary
At the Peter MacCallum Cancer Centre, our patient care is delivered via a tumour stream model. This means that a patient’s care is coordinated and provided by a multi-disciplinary team of people who have specialist knowledge of a specific type of cancer (e.g. breast or lung) and associated patient pathways.
The patient navigation team is a large team of 30-40 administrators, who work within the tumour streams alongside expert clinicians in those specific disciplines.
Patient Navigators are specialist administrative professionals responsible for coordinating and facilitating a patient's journey through Peter Mac. Acting as a central point of contact for patients, clinicians, and multidisciplinary teams, they ensure diagnostic investigations, treatment appointments, clinic reviews, and associated care requirements are appropriately sequenced, coordinated, and communicated.
Patient Navigators use critical thinking, problem-solving, and service knowledge to proactively identify and resolve scheduling, pathway, and administrative issues, supporting timely access to care and an optimal patient experience. They play a key role in multidisciplinary coordination, including appointment management, multidisciplinary meeting (MDM) administration, imaging and documentation management, and facilitation of complex clinical workflows. Using sound judgement and a patient-centred approach, Patient Navigators actively triage enquiries, coordinate care activities, and troubleshoot barriers to ensure patients receive timely, seamless, and well-coordinated care.

Essential Requirements
A patient-centered approach and a genuine commitment to providing high-quality support to people affected by cancer within a public healthcare setting.
Well-developed written and verbal communication skills, with the ability to communicate clearly, professionally and compassionately with a diverse range of patients, carers and multidisciplinary staff.
Demonstrated ability to learn and confidently use complex information technology systems and adapt to new technologies and processes.

Coordinate appointments, referrals and patient pathways in collaboration with the multidisciplinary team.
Monitor and action work queues within the Electronic Medical Record (EMR) and other clinical information systems.
Coordinate and manage the administrative requirements of the allocated tumour stream to support safe, timely and efficient patient care.
Prioritise competing work demands while maintaining accuracy and meeting service expectations.
Provide cross-cover for other Patient Navigator portfolios as required to ensure continuity of service.
Support the implementation of new systems, workflows and organisational initiatives, including Electronic Medical Record (EMR) enhancements.
Participate in team meetings and contribute positively to achieving departmental and organisational objectives.
