The Chronic Care Program is a multidisciplinary service that provides predominantly virtual care for people living with chronic disease. The service supports patients to effectively self-manager their conditions while enhancing care coordination, with the aim of reducing hospital admissions. The team delivers both face-to-face and virtual care in the community, offering a range of supports including care navigation, care coordination, and health coaching interventions. The service works with Aboriginal and non-Aboriginal people of all ages across the Illawarra Shoalhaven district who are living with chronic and life-limiting illness. The focus is to enable patients to live independently and comfortably at home for as long as possible. This is supported through access to nursing and allied health professionals to assist with symptom management and overall wellbeing.
This position delivers patient-centred care through a combination of telehealth, virtual communication platforms, and face-to-face interactions, ensuring flexible and responsive service delivery tailored to individual needs.
