About the role

This is a unique opportunity to contribute to improving heart health outcomes for Aboriginal and Torres Strait Islander peoples through the innovative RECARD project. Based within Cardiology at Royal Brisbane and Womens Hospital (RBWH), you will play a key role in supporting patients as they navigate their recovery journey from hospital back to community care. Working as part of a multidisciplinary team, you will act as a cultural connector - helping ensure care is respectful, accessible and aligned with patients' cultural needs and lived experiences.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

Provide support to Aboriginal and Torres Strait Islander patients, families and/or carers in Cardiology.
Work collaboratively with the RECARD team and the community services to facilitate an effective and efficient patient journey for Aboriginal and Torres Strait Islander patients, families and/or carers including:
The planning, implementation and delivery of care and education for people with cardiac disease.
Advise the RECARD team regarding culturally appropriate education materials for Aboriginal and Torres Strait Islander patients, families and/or carers.
Work collaboratively with the RECARD team to liaise between Aboriginal & Torres Strait Islander patients and their families, community groups and organisations to increase their capacity to be more involved in their own health care.
