Across Australia Just Better Care provides in-home aged care and disability support services, supporting people to live independently.

We’re seeking a Clinical Team Leader (Registered Nurse) to join our dedicated team at Just Better Care Mid North Coast. This is a unique opportunity to combine clinical expertise with team leadership, ensuring the delivery of safe, high-quality and person-centered care across our community.

As a Clinical Team Leader, you will:

Lead, supervise and support a team of nurses and Community Support Professionals fostering a collaborative team environment.
Oversee the development, implementation and review of individual care and health plans, ensuring alignment with best practice and participant goals
Conduct comprehensive clinical assessments and respond proactively to changing health needs and risks.
Coach, mentor and train staff in clinical skills and competencies, fostering a culture of continuous improvement.
Collaborate with participants, families, and health professionals to deliver integrated, respectful, and outcome-focused services.
Monitor risks, respond to changing participant needs, and ensure compliance with aged care, disability, and health standards
Contribute to organisational performance through incident management, quality improvement initiatives, and strategic service delivery.

Duties and responsibilities

Working with or under the guidance of medical practitioners and Allied Health professionals
Medication Administration and Complex Clinical Care eg. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more
Monitoring and communicating on the customer’s wellbeing and reporting on progress of customers
Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
Providing training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as required
Conducting skills assessments of Community Support Professionals performing complex care services as required
Demonstrating a commitment to best practice and evidence of continued professional development
Participating in staff meetings and attending training opportunities when required

About you

We are looking for an experienced and motivated leader who has:

Current AHPRA registration as a Registered Nurse.
Previous experience within aged care, disability or community services.
Demonstrated team leadership experience – including staff supervision, support and training.
Strong clinical decision-making skills and ability to provide hands-on care when required.
Excellent communication, relationship-building and problem-solving skills.
Commitment to person-centred, trauma-aware and culturally safe care.
A current NDIS Worker Screening Check, or willingness to obtain one.
