About the Position
Social Prescribing in General Practice aims to improve patient outcomes by addressing social, emotional and wellbeing needs.
As a Wellbeing Practitioner, your responsibilities will include:
- Assess client wellbeing needs, risks and safety using approved tools, and co‑develop tailored wellbeing and safety plans.
- Provide client‑centred support using solution‑focused approaches, including practical assistance, education and skill building.
- Deliver information, supported referrals and warm linkages to appropriate services, groups and community activities.
- Advocate for clients to access necessary supports such as housing, financial, legal, health and counselling services.
- Collaborate with internal teams, general practice staff and external stakeholders to ensure coordinated, culturally safe service delivery.
- Maintain accurate records, contribute to data reporting, participate in team processes, professional development and quality improvement activities.

About You
To be successful in this role you will have:
- A bachelor’s degree in social work or psychology, with eligibility for relevant professional membership or registration (e.g. AASW or Psychology Board of Australia). Alternative qualifications considered: Nursing with a postgraduate mental health qualification, or Occupational Therapy, with eligibility for relevant AHPRA/NMBA registration.
- Previous experience in providing counselling and therapeutic interventions.
- Sound knowledge of mental illness, local community resources and connections. Demonstrated critical thinking, reflective practice skills and abilities.
- Knowledge of trauma-informed and culturally safe practices.
- Ability to work independently and collaboratively.
- Demonstrated ability to work with and be sensitive to the differing needs of people from various cultural groups.
- High level of communication and negotiation skills, both verbally and in writing.
- Effective organisational and time management skills.
- Demonstrated ability to work in a highly confidential manner.
- High level of ability in the use of Microsoft Office and Information Technology.
Requirements of the role:
- Hold (or willingness to obtain) a current DHS Working With Children Check *
- Hold (or a willingness to obtain) a National Criminal History Record Check (Police Check) *
- Hold a current South Australian Driver’s License (minimum Provisional License)
