About the role:
An exciting opportunity exists for an experienced and motivated Allied Health Assistant Grade 3 to join the Community Based Transition Care Program (TCP) within Western Health at Home.
Working under the guidance of Allied Health Professionals, you will support older people transitioning from hospital to home by delivering evidence-based therapy programs and health promotion activities that maximise independence, improve functional outcomes and support safe, sustainable discharge plans. This autonomous community-based role involves providing care in patients' homes across the Western Health catchment and offers the opportunity to work as part of a highly skilled multidisciplinary team committed to helping people live healthier days at home.
Key responsibilities:
- Deliver prescribed therapy, rehabilitation and health promotion programs under the direction of Allied Health Professionals.
- Support older adults to improve function, independence and wellbeing in their home and community environments.
- Monitor patient progress, identify clinical concerns and communicate effectively with supervising clinicians.
- Provide clinical support, mentoring and supervision to Grade 1 and 2 Allied Health Assistants and students as required.
- Maintain accurate clinical documentation, workload statistics and contribute to quality improvement activities within the service.
About you:
- Certificate IV in Allied Health Assistance or equivalent qualification.
- Experience working with older adults and/or people with complex health conditions.
- Strong communication, organisational and time management skills with the ability to work independently.
- A compassionate, person-centred approach and commitment to delivering high-quality care.
- Current Victorian Driver Licence and confidence travelling throughout the community to provide patient care.
