Are you passionate about aged care and ready to make a meaningful impact? At BaptistCare, we’re looking for a dedicated Care Assessment Coordinator to join our Residential team at Morven Gardens in Leura. This is your opportunity to be part of an organisation that truly makes a difference in the lives of others.

About the Role:

As a Care Assessment Coordinator, you will play a vital role in coordinating the AN-ACC (Australian National Aged Care Classification) process, leading pre-admission assessments, and ensuring optimal funding outcomes. You’ll work closely with our Residential Manager and Funding Consultants to support residents' care journeys and uphold the highest standards of aged care.

Your Team- Admissions and Funding Team – Support Services

This division helps the organisation achieve its purpose by providing the business with integrated and standardised management frameworks that are customer driven and achieve outcomes and value for its stakeholders through our planning, consultation, and strategy development. To provide the highest quality Aged Care Services that are consistent with BaptistCare purpose and values. To enable BaptistCare Aged Care Services to meet clients’ needs and the quality of service is recognized as setting industry standards.

About You:

To be considered, you will have:

A qualification as a Registered Nurse, along with experience in aged care and knowledge of the AN-ACC
Strong interpersonal and communication skills
Proficiency in MS Office and clinical documentation systems
Ability to lead, train, and support staff
Excellent time management and problem-solving skills
Experience with ACFI and quality improvement tools - desirable
