About the Role
As a Support Coordinator at Tuley, you will work collaboratively with participants, families, and providers to build capacity, create meaningful connections, and coordinate supports that empower individuals to live their best possible life.
Core Responsibilities
Build empowering, respectful relationships with participants and their support networks
Coordinate responsive, individualised supports tailored to diverse needs
Apply person-centred and trauma-informed practice
Navigate the NDIS framework and guidelines with confidence
Research and develop innovative support options
Lead and coordinate multiagency meetings and reviews
Conduct risk analysis and ongoing support planning
Prepare high-quality written reports and documentation
Maintain clear, timely, and transparent communication with stakeholders
What You Bring
Minimum 1 year of experience in Support Coordination or Case Management within the disability sector.
Thorough understanding of the NDIS framework, including funding mechanisms and legislative requirements.
Experience working with people with diverse backgrounds and support needs
Strong ability to work independently and collaboratively
Creative problem-solving and initiative in planning supports
Commitment to reflective practice, learning, and continuous improvement
Full Australian Driver Licence and access to a comprehensively insured vehicle for client visits and community engagement.
Strong written and computer skills
Ownership and accountability in your work
Essential Requirements
Minimum 1 year experience in Support Coordination or Case Management
Strong understanding of the NDIS framework
Commitment to person-centred and trauma-informed approaches
High-level report writing skills
NDIS Worker Screening Check
Working With Children Check
Full Driver Licence + access to a comprehensively insured vehicle
Australian working rights
Flexibility to work from home
Desirable Qualifications
Tertiary qualification in a relevant field (e.g., Social Work, Psychology, Disability, Nursing, Occupational Therapy, Teaching)
