About the Role
In this role, you will play a key part in delivering the Community Connections and Housing Support for the Aged programs, providing intensive, person‑centred support to people experiencing or at risk of homelessness across the Casey, Cardinia and Dandenong LGAs.
You will work with adults who have complex needs and are living in rooming houses, caravan parks, other low‑cost accommodation, or who are sleeping rough. Through a trauma‑informed, outreach‑based model, you will deliver short to medium‑term case management that helps clients stabilise their housing, improve wellbeing, and build pathways toward independence.
You will also support older people living in social housing, helping them maintain their tenancy, strengthen independence, and connect with essential health, welfare and community supports. Central to this role is the creation of holistic, goal‑oriented support plans and strong advocacy to ensure clients achieve positive, sustainable outcomes.
Role Responsibilities
Provide short to medium‑term, client‑centred case management through outreach, including home visits, public spaces and community hubs, to build trust, assess needs and deliver tailored support.
Conduct detailed assessments to understand clients’ circumstances, risks and strengths, and develop targeted, achievable support plans that address their holistic needs.
Facilitate and advocate for clients’ access to appropriate services, including health, counselling, employment, education, recreation, housing and substance use supports.
Build and maintain strong relationships with service providers, partner agencies and key community stakeholders to enhance collaboration and optimise client outcomes.
