About the Role
An exciting opportunity is now available for a Residential Aged Care Liaison (RACL) to join our team at Casey Hospital on a permanent part-time basis (0.8 EFT). This role is central to supporting patient flow across acute and subacute settings and sits within the Social Work Service, operating Monday to Friday during standard business hours. This opportunity is open to Social Work & Nursing disciplines.
As the Residential Aged Care Liaison, you will work closely with patients, families, carers, and multidisciplinary teams to support safe and timely transitions into residential aged care, supported accommodation, and Transitional Care Programs. You will play a key role in navigating aged care pathways for older people, as well as younger people with disabilities who meet eligibility criteria for residential aged care services.
Working collaboratively with Social Work, Allied Health, medical teams, and key stakeholders including the Aged and Rehabilitation Access Service, you will provide expert guidance, advocacy, and coordination to achieve the best possible outcomes for patients and families.
For Social Workers, the role operates in line with the professional standards and ethical framework of the Australian Association of Social Workers (AASW), including credentialing, supervision, and scope of practice requirements.
About You
You are an experienced Grade 2 Social Worker or Registered Nurse who is passionate about supporting vulnerable patients through complex discharge and care planning pathways. You will bring:
- Strong knowledge of the Aged Care Act and Residential Aged Care pathways
- Experience delivering specialised clinical services within healthcare settings
- Excellent stakeholder engagement and multidisciplinary collaboration skills
- Highly developed written and verbal communication abilities
- A patient-centred, compassionate, and solutions-focused approach
- The ability to work autonomously while contributing positively to a supportive team environment
- A desire to further develop your leadership capability and clinical expertise
Qualification & licence requirements include:
- Qualification in Social Work with eligibility for membership with the Australian Association of Social Workers (AASW)
- Evidence of overseas qualifications (if applicable)
- OR Nursing qualifications with current professional registration
- Current Victorian Driver’s Licence
