This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.

About The Opportunity
As part of a community-based multidisciplinary Allied Health team, you will deliver person-centred care to clients and their families across a range of community settings. The role works closely with ward-based social workers to support continuity of care for outpatients who are admitted to hospital and, where appropriate, provides inpatient support to assist with ongoing care planning and coordination.
Working within a dedicated interdisciplinary team, you will contribute to the delivery of high-quality, evidence-based care in a dynamic and collaborative environment.
Key responsibilities include:

Conducting comprehensive psychosocial assessments and providing counselling and emotional support to patients and carers navigating life-limiting illness and end-of-life care

Undertaking case management, care planning, advocacy, liaison, and referrals to ensure timely access to appropriate community and support services

Collaborating closely with interdisciplinary colleagues, senior clinicians, and managers to deliver responsive, proactive, and effective social work services

Participating in service development, quality improvement, evaluation activities, and the education and supervision of staff and students

Working collaboratively with social work colleagues to ensure equitable and timely service delivery across multiple geographical locations and services within CCLHD
This position involves providing clinical services across various sites within the Central Coast Local Health District and offers the opportunity to be part of a supportive team committed to excellence in end-of-life care.

About You

Qualification of a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number) with demonstrated understanding of the current AASW Practice Standards as they relate to the position and a commitment to maintaining a professional skill and knowledge base and current drivers licence.

Demonstrated well-developed understanding of the psycho-social impact of acute and chronic health conditions and in the range of related social work interventions required in community social work clinical practice.

Demonstrated skills and experience in working with a broad range of NSW Health, government and non-government organisations to achieve desired clinical outcomes for service and clients

Demonstrated effective verbal, written and interpersonal communication skills including the ability to adapt communication style to suit different audiences and modalities (individual, family and groups).
