Job Description
Permanent part-time role - 60.8 hour per fortnight position
We are seeking a compassionate and detail-oriented Social Worker to join our team in Joondalup, Australia. In this role, you will provide comprehensive social work support to individuals and families navigating complex life circumstances, health challenges, and transitions. Working collaboratively within a multidisciplinary environment, you will deliver person-centered interventions that empower clients to achieve their goals and improve their overall wellbeing. Your expertise in case management, crisis intervention, and resource coordination will be instrumental in making a meaningful difference in the lives of those we serve.
- Conduct comprehensive assessments and evaluations to identify client needs, strengths, and barriers to wellbeing
- Develop and implement individualized care plans utilizing evidence-based social work interventions and best practices
- Coordinate complex discharge planning and care transitions, ensuring seamless continuity of services
- Provide crisis intervention and support during times of acute need, demonstrating resilience and decisive action
- Maintain detailed, organized case documentation and records in compliance with organizational and regulatory standards
- Collaborate effectively with healthcare professionals, community agencies, and support networks to optimize client outcomes
- Connect clients with appropriate community resources, services, and referral pathways tailored to their specific needs
- Advocate transparently for client rights and wellbeing while maintaining professional boundaries and ethical standards
- Conduct follow-up assessments and monitor progress toward established goals, adjusting interventions as needed
- Participate in team meetings and contribute analytically to multidisciplinary care discussions
- Demonstrate cultural competency and inclusive practices that respect diverse backgrounds and perspectives
- Manage time efficiently while maintaining high-quality service delivery across a diverse caseload
About you
1.Bachelor of Social Work (or equivalent).
2.Eligible for full membership of Australian Association of Social Workers.
3.Demonstrated ability to utilise a variety of Social Work interventions.
4.Well developed skills in:

Problem solving and ethical decision making

Organisation and time management
-
Communication (written and verbal).Ability to use initiative, self motivation, work as a team member with minimal supervision.
5. Ability to work effectively within a multi-disciplinary team.
6.Demonstrated experience in medical Social Work practice.
7.Knowledge of community resources services to meet client needs.

Remuneration
$44.72 - 61.88 + superannuation based on experience level and qualifications, exclusive of penalties or allowances. (RHC WA PACT Enterprise Agreement: Professional Level 1 (WA PACT))
