The Palliative Care Social Worker will be a valued member of the Community Palliative Care Team to undertake comprehensive psychosocial assessments, counselling, advocacy, care coordination and supportive interventions for patients, carers and families receiving palliative care services. Working within a multidisciplinary team, you will contribute to the delivery of compassionate, person-centred care across community and outpatient settings.
We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity.
About You
- Bachelor of Social Work or equivalent qualification, with eligibility for membership of the Australian Association of Social Workers (AASW).
- Demonstrated experience in social work practice within healthcare, community health, palliative care or a related clinical setting.
- Experience undertaking psychosocial assessments, counselling, advocacy and case management.
- Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, carers and stakeholders.
- Demonstrated ability to work collaboratively within a multidisciplinary team.
- Strong organisational and time management skills, with the ability to manage competing priorities.
- Commitment to delivering high-quality, person-centred care.
- Current Victorian or NSW Driver Licence.

Employment Type: This is a permanent part-time (32 hours per week) opportunity.
