Senior Social Worker – General Medicine Acute | SALHN
Join the General Medicine Acute team at Flinders Medical Centre as a Senior Social Worker, delivering specialist psychosocial support to patients navigating some of the most complex moments of their lives. Working part-time within one of South Australia's busiest tertiary hospitals, you'll lead discharge planning, provide crisis intervention, and collaborate with a respected multidisciplinary team. This is a long-term contract role running through to March 2028 — offering real stability, genuine clinical scope, and the chance to do meaningful work every shift.
About the role:
Reporting to the Social Work Manager, service provision includes assessment, consultation, intervention and review, education, liaison with community agencies, organisation of appropriate resources and management of discharge.
Duties for the role will include:
- Conducting psycho-social assessments and intervening with complex patients admitted to FMC using a range of well-developed therapeutic social work skills,
- Working with the multi-disciplinary team, patients and their families to achieve timely, safe and effective outcomes for patients including discharge plans for patient with complex psycho-social factors that impact on discharge,
- Providing a consultancy service to staff and the multi –disciplinary team in their area of expertise and in the management of complex cases,
- Attending internal and external clinical forums and meetings as a representative of the Social Work department and/or the clinical area of expertise to provide specialist social work opinions and knowledge,
- Providing leadership, consultancy and advice to management and other health professionals,
- On service delivery, development and practice within Social Work and Counselling Services
About you:
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers,
- Demonstrated well developed interpersonal skills that foster trust, respect, confidence and cooperation with colleagues, patients, families, multi-disciplinary team members and, senior staff,
- Demonstrated ability to develop a range of partnerships, networks and relationships with key stakeholders within and external to the organisation to address complex psycho-social issues and achieve desired outcomes,
- Demonstrated ability to demonstrate leadership skills by thinking and working strategically, proactively and independently to develop creative and effective solutions to problems and issues that can be evaluated,
- Extensive experience working with patients and families as a social worker in a specialty area within the Acute /Sub Acute Care Setting,
- Extensive experience in providing clinical management to complex and difficult casework,
- Experience in leading quality improvement, service evaluation activities, policy development
