Kurunjang Secondary College is a government secondary school in Melbourne's west, serving approximately 920 students from Years 7 to 12. Kurunjang delivers targeted literacy instruction through the Kurunjang Literacy Program (KLP) and supports numeracy through scaffolded intervention. The Year 6 to Year 7 Enhanced Transitions Partnership Coordinator will strengthen transition pathways between Kurunjang Secondary College and key feeder primary schools, with Kurunjang Primary School as an initial priority.
The role is designed to move transition beyond an enrolment process and into a deliberate, evidence-informed partnership model that supports students earlier, strengthens family connection, and improves learning, wellbeing, attendance, and engagement outcomes as students move from primary to secondary school.
Working under the direction of the Transitions and Pathways Leading Teacher, the coordinator will support stronger collaboration between primary and secondary staff, earlier identification of student needs, improved transition documentation, and targeted preparation for students likely to require additional support in Year 7.

Reporting and Accountability
The position will work under the day-to-day direction of the Transitions and Pathways Leading Teacher. As this is a new role, it will operate under the strategic oversight of the principal for the first year, in consultation with relevant college leaders. The role will also work closely with the Disability and Inclusion Team, House Leaders, Wellbeing Team, Assistant Principals, primary school partners, and other staff involved in Year 6 to Year 7 transition.

Key Responsibilities
Partnership and communication
Build and maintain a strong working partnership with key staff at Kurunjang Primary School and develop relationships with other main local feeder primary schools as required.
Establish regular communication with nominated primary school staff and relevant Kurunjang Secondary College leaders.
Support the development of improved transition processes, timelines, and communication routines.
Help ensure cohort-level themes and individual transition priorities are communicated to relevant KSC teams early enough to support proactive planning.
Early identification and transition planning
Work with relevant KSC and primary school staff to support earlier identification of students who may require additional planning for a successful transition to Year 7.
Assist with the collation of individual and cohort information and data, where relevant.
Programs, activities, and student connection
Under the guidance of the Transitions and Pathways Leading Teacher, and in collaboration with other key staff, assist with planning and implementing programs and activities for Year 6 students throughout the year.
Assist with the planning and implementation of a pilot mentoring or connection program for Year 6 students, where agreed.
Support the implementation of additional transition days, visits, or targeted activities throughout the year.
Family engagement
In collaboration with key KSC staff, help build early and positive relationships with Year 6 to Year 7 enrolling families, particularly where students may require additional transition support.
Help families understand secondary school expectations, available supports and key transition processes.
Identify families who may require more personalised transition support and provide this information to the appropriate KSC leader or Assistant Principal/s.
Help families feel known and connected with KSC staff before Year 7 commencement where relevant.
Inclusion, wellbeing, and support coordination
Liaise regularly with House Leaders, the Wellbeing Team and the Disability and Inclusion Team to support proactive planning for prospective Year 7 students.
In partnership with the Disability and Inclusion Team, attend Semester 2 SSGs at Kurunjang Primary School and other primary schools for prospective Year 7 students, as required, to provide administrative and operational support.
Support Disability Inclusion processes, including preparation and organisation of documentation for Disability Inclusion Profile meetings for Year 7 students.
Documentation, enrolment, and administrative support
Under the guidance of the Transitions and Pathways Leading Teacher, undertake all Grade 6 to Year 7 enrolments including one-on-one meetings.
Under the guidance of the Transitions and Pathways Leading Teacher, and in collaboration with the Disability and Inclusion Team, House Teams and Wellbeing Team, document relevant transition data and key communication with schools, families, and external agencies.
Maintain accurate and timely transition records on Compass, including through early Year 7 groups created outside the official Home Group allocation, where required.
Support prospective students and families with enrolment-related documentation.
Support Accounts Receivable processes for State Schools' Relief as part of the Year 6 to Year 7 enrolment process.
Attend relevant transition meetings and information evenings, as required.
Undertake other transitions, enrolment and inclusion-related administrative duties as directed by the principal or nominee.
