Full Job Description

Your new role:
As a Social Worker - Accommodation Support, you will provide high-quality Social Work services to patients experiencing homelessness or housing instability across Logan Hospital. You'll undertake clinical assessment, therapeutic intervention, patient education and discharge planning while working collaboratively with patients, families, healthcare professionals and external agencies.

Key Responsibilities

Provide Social Work assessment, intervention and discharge planning for patients experiencing homelessness or housing risk.
Manage a diverse caseload across the Emergency Department and inpatient wards.
Work collaboratively with multidisciplinary teams, patients, families and external agencies.
Contribute to student education, quality improvement, research and service development.

About you:
You are a compassionate and capable Social Worker who can build strong relationships with people experiencing complex social circumstances. You'll bring sound clinical judgement, effective communication and a commitment to delivering evidence-based, person-centred care.

Skills & Experience

Tertiary qualification in Social Work and eligibility for AASW membership.
Ability to deliver high-quality, evidence-based Social Work assessment and intervention.
Strong communication, collaboration, negotiation and interpersonal skills.
Commitment to professional development, quality improvement and patient-centred care.
