What You Will Be Doing
As a Registered Nurse within the Hospital in the Home team, you will provide comprehensive clinical care to patients receiving hospital-level treatment in their own homes following a recent hospitalisation.
Working as part of a multidisciplinary team, you will undertake nursing assessments and deliver a range of clinical services including:
- Comprehensive nursing assessments and clinical observations
- Medication administration and reconciliation
- Wound care and ongoing treatment management
- Collection of pathology specimens
- Patient and carer education
- Liaison with medical officers, general practitioners and other healthcare providers
- Monitoring patient progress and escalating concerns where required
This role requires a high level of clinical judgement, adaptability and independence while maintaining a strong focus on patient safety and quality care.
About you
We are seeking an experienced Registered Nurse who is passionate about delivering high-quality care in a community setting.
You will bring:
- Current registration as a Registered Nurse with AHPRA
- Extensive clinical nursing experience
- Strong assessment and clinical decision-making skills
- Excellent communication and interpersonal skills
- Demonstrated ability to manage competing priorities and work autonomously
- Confidence in providing care independently within patients' homes
- A patient-centred approach and strong advocacy skills
- The ability to quickly assess situations and respond appropriately to changing clinical needs
