About you
Our ideal candidate will be someone who can demonstrate the following:
Contemporary clinical knowledge in the areas of specialised acute and chronic respiratory disease management, in a community setting in collaboration with individuals, peers and the healthcare team to achieve improved health outcomes across Preventative Health Services.
Leadership in the coordination of nursing and provision of health services across the continuum of care, delivering competent nursing care in complex situations and ability to coach patients to adopt self-management strategies.
Ability to adopt digital technology and utilise digital care solutions and telehealth to provide a co-ordinated approach to delivering care closer to home.
Advanced clinical skills in the identification and adoption of innovative clinical practice models and work practices that develop partnerships to support the continuum of care including contemporary technologies (IT,ICT).
Advanced level delivery of interpersonal (problem solving/conflict resolution), communication (verbal/written) and time management skills whilst demonstrating autonomous, sound, high level conceptual and critical thinking capabilities.
Mandatory Requirements:
Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (Ahpra). Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to the commencement of clinical duties.
