About the Opportunity

The Complex Care and Frequent User Management (CCFUM) Program is a statewide clinical initiative aimed at improving outcomes for patients with chronic and complex health conditions including those who frequently present to NSW Ambulance.

The Coordinator, Supportive Care will support the strategic design, implementation, and administration of an older person supportive care pilot within the Complex Care and Frequent User Management Program. This includes coordinating stakeholder engagement, managing program resources, and contributing to quality improvement and service planning to enhance care for complex older patients with life limiting illness.

About You

Demonstrated experience in coordinating health or social care programs, including understanding of supportive care principles and the needs of older patients with complex health and social care requirements.
Ability to interpret and apply legislative requirements, organisational policies, and procedures to ensure compliance and governance in program delivery.
Proven experience in high-level system administration, including managing program documentation, workflows, and reporting.
Advanced digital and computing skills, including proficiency in platforms such as Microsoft Office, and other relevant systems.
Highly developed interpersonal and communication skills, enabling effective engagement with diverse stakeholders, including vulnerable cohorts, while working independently and collaboratively.
Understanding of stakeholder engagement strategies, particularly for vulnerable populations, and ability to design initiatives that address priority needs.
Strong organisational and time management skills, with the ability to manage competing priorities and meet deadlines in a dynamic environment.
Excellent written communication skills, including attention to detail and the ability to prepare reports, resources, and correspondence for internal and external audiences.

Conditions

There is no entitlement to Transferred Employees Benefit (TEB) or travel allowance associated with this opportunity.
No NSW Ambulance accommodation is provided.
Employees currently undertaking an Extended Care Paramedic or Intensive Care Paramedic consolidation periods are eligible to apply however will need to complete all training and return of service obligations before accepting any relief/acting opportunities.
Employees currently undertaking a Special Operations Team (SOT) consolidation period are not eligible to apply for positions other than SOT positions. This includes positions offered via merit selection, transfer lists or later transfer mechanism, expressions of interest or secondments.
