Full Job Description

Join our growing team and make a real difference in the lives of older Australians!

We are looking for a compassionate and motivated Case Manager to join our Eastwood office on a full-time basis. In this role, you will work closely with clients receiving Support at Home (SaH) Packages, ensuring they receive quality, person-centred services that help them live independently at home.

What you’ll do:

Manage a portfolio of Support At Home clients.
Develop, review and monitor care plans to meet client goals.
Provide advice and guidance to clients and their families.
Coordinate the delivery of all services by Support Staff, including scheduling/rostering
Monitor package budgets and ensure compliance.
Work Health and Safety issues/Risk Management
A mixture of being in the office and being out in the community visiting clients

What you’ll bring:

Qualification in Nursing, Community or Human Services, Case Management, Psychology, Social Work, Allied Health or Rehabilitation Case Management.
Experience in case management and aged care.
Strong knowledge of Support at Home funding and Aged Care Quality Standards.
Great communication and problem-solving skills.
Excellent documentation and computer skills
Police Check / NDIS Worker Screening Check (or willing to obtain).
Unrestricted current Australian driver licence and reliable car.
