Support and advocate for Aboriginal patients, build community links and create culturally safe care pathways across Westmead Hospital.

Remuneration: $65,012 - $93,971 per annum
Hours Per Week: 38 (Job Share may be considered for part time hours)

Westmead Hospital is a major tertiary referral hospital within Western Sydney Local Health District, delivering complex, high-quality care to one of Australia’s most diverse communities. The Aboriginal Liaison Officer role sits within Social Work, ensuring culturally safe, respectful and holistic care for Aboriginal and Torres Strait Islander patients and families.

You will work in a supportive, values-driven environment that recognises the importance of culture, community and partnership in improving health outcomes.

What you'll be doing
As an Aboriginal Liaison Officer, you will play a vital non-clinical role in supporting Aboriginal and Torres Strait Islander patients during their hospital journey, ensuring culturally safe, coordinated and person-centred care. You will:

Advocate for Aboriginal patients and families while they receive inpatient care
Support discharge planning and continuity of care through referrals and community links
Work alongside clinical and non-clinical staff to identify and respond to patient needs
Build and maintain strong connections with local Aboriginal communities and services
Contribute to Aboriginal health programs, education initiatives and cultural safety goals

Essential Criteria:

Qualification: Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care in which the Aboriginal Health Worker works
Evidence of Diversity claim if applicable (state which category): Aboriginal and / or Torres Strait Islander (to be confirmed at interview)

Selection Criteria:

Ability to undertake health/social needs assessment of Aboriginal patients and to advocate on their behalf in the health system

Demonstrated understanding of the social, psychological and financial implications of hospitalisations for Aboriginal and / or Torres Strait Islander patients

Effective interpersonal skills with a demonstrated ability to develop and maintain relationships with clients.

Demonstrated experience working as part of a multidisciplinary team to provide culturally responsive care co-ordination or case management services to Aboriginal and / or Torres Strait Islander clients.

Knowledge of a range of community-based support services and programs to assist Aboriginal patients and their families in addressing their health care needs

Understanding of the management of patient complaints and patient privacy and escalation processes.

Competence in the use of databases, electronic documentation, and computer based administrative systems

Demonstrated skill in the analysis of complex issues, problem solving, formulating new approaches and implementing comprehensive change programs and improvement strategies, including the ability to develop and monitor outcomes
