About The Role: We are seeking to recruit an enthusiastic, experienced Disabilities Services Officer to provide essential care and support to residents in our aged care facilities across the following tasks:

Assessing clients' needs and developing and implementing support programs and plans to best support those needs;
Developing individualized service plans in collaboration with clients, their families, and other stakeholders;
Coordinating support services, ensuring clients' receive the required care that best meets their assessed needs;
Providing support services to clients through assessment, case-management, care planning, budget management, and service coordination;
Monitoring care package client costs, ensuring services are provided within each clients' budget, package and plan levels;
Monitoring and reporting on the progress of clients and referring clients to agencies that can provide additional help;
Assisting clients with challenging behaviours through participation in the development and implementation of individually designed behavioural management plans;
Coordinating social support activities for clients to participate in community events and activities;
Building and maintaining positive relationships with clients and their families and participating in client case meetings;
Maintaining accurate and timely records and ensuring all relevant documentation is completed.

Requirements: The ideal candidate will hold a relevant Diploma or higher in a related field with 1-2 years of work experience. At least three (3) years of relevant experience may be accepted as a substitute for the formal qualification.
